All Name and National Identity Card (NIC) amendment requests must be submitted exclusively using the <strong>AMD-1 Form<\/strong>.<\/p>\n<\/div>\n<p><!-- Main Introduction --><\/p>\n<p style=\"font-size: 1.05em; color: #333;\">To ensure a more streamlined, accurate, and efficient service for employers and employees, the Employees\u2019 Provident Fund (EPF) Department of the Central Bank of Sri Lanka introduced the standardized <strong>AMD-1 Form<\/strong> on 03 June 2026 for processing Name and NIC amendments in the EPF database.<\/p>\n<p><!-- Key Submission Guidelines Box --><\/p>\n<div style=\"background: #f8fafc; border: 1px solid #e2e8f0; border-radius: 8px; padding: 20px 24px; margin: 25px 0;\">\n<h3 style=\"color: #1e3a8a; margin-top: 0; margin-bottom: 15px; font-size: 1.2em; border-bottom: 2px solid #cbd5e1; padding-bottom: 8px;\">Mandatory Requirements for Submissions<\/h3>\n<ul style=\"padding-left: 20px; margin-bottom: 0; space-y: 8px;\">\n<li style=\"margin-bottom: 10px;\"><strong>Mandatory AMD-1 Form:<\/strong> All amendment requests must be submitted using the original AMD-1 Form. <em>(A company cover letter\/letterhead is no longer required).<\/em><\/li>\n<li style=\"margin-bottom: 10px;\"><strong>Required Supporting Documents:<\/strong>\n<ul style=\"list-style-type: circle; margin-top: 5px; padding-left: 20px;\">\n<li>A clear copy of the member\u2019s <strong>National Identity Card (NIC)<\/strong>, duly certified by the employer.<\/li>\n<li>A clear copy of the member\u2019s <strong>B Card<\/strong>, duly certified by the employer.<\/li>\n<\/ul>\n<\/li>\n<li style=\"margin-bottom: 10px;\"><strong>Physical (Hard Copy) Submission Only:<\/strong> The AMD-1 Form and supporting certified copies must be submitted in <strong>hard copy<\/strong>.
