Effective from 01 October 2026, traditional amendment request letters on company letterheads will strictly no longer be accepted. All Name and National Identity Card (NIC) amendment requests must be submitted exclusively using the AMD-1 Form.
To ensure a more streamlined, accurate, and efficient service for employers and employees, the Employees’ Provident Fund (EPF) Department of the Central Bank of Sri Lanka introduced the standardized AMD-1 Form on 03 June 2026 for processing Name and NIC amendments in the EPF database.
Mandatory Requirements for Submissions
- Mandatory AMD-1 Form: All amendment requests must be submitted using the original AMD-1 Form. (A company cover letter/letterhead is no longer required).
- Required Supporting Documents:
- A clear copy of the member’s National Identity Card (NIC), duly certified by the employer.
- A clear copy of the member’s B Card, duly certified by the employer.
- Physical (Hard Copy) Submission Only: The AMD-1 Form and supporting certified copies must be submitted in hard copy. Electronic submissions via email or soft copies will not be accepted.
