Amendments refer to correcting discrepancies in EPF member records, including name, NIC, and contribution details.

  • EPF Department – Whiteaways Building, Colombo 01
  • Regional Offices: Matara, Anuradhapura, Matale, Trincomalee, Kilinochchi, Nuwara Eliya

  • A. Member Name / NIC Amendments
  • B. Member Account Amendments

Required Documents

Mandatory

  • AMD 1 Form⬇ Download Form
  • Certified Copy of NIC (Locals)
  • Certified Copy of Passport (Foreigners)
  • Certified B Card

If Applicable

  • Marriage / Divorce Certificate (In a case where the name has changed based on marriage)
  • Birth Certificate (In a case where the name in the 13th column has changed)
  • DRP Letter (In case where NIC number is completely changed)
  • Contribution History⬇ Download Statement
  • Department of Labour recommendation
