Determine which specific category your amendment falls under. The required procedures vary based on the classification:

  • A. Member Name Amendments: Fixes discrepancies in initials, spelling mistakes, full names denoted by initials, name changes after marriage, or updates according to the birth certificate.
  • B. NIC Number Amendments: Corrects typographical errors in the NIC number, handles changes from an old NIC to a new NIC format, or separates identical NIC numbers mistakenly assigned to multiple members.
  • C. Member Account Amendments: Resolves contribution discrepancies, removes dummy numbers generated due to past mismatches, or manages contributions credited to already refunded accounts.

Applications for updates to the master database can only be submitted or authorized by the following parties:

  • Active Members: The individual member via their active employer.
  • Registered Employers: The current operating employer certifying the identity of the staff member.
  • Legal Representatives / Heirs: In the case of a deceased member, an authorized representative supported by advice from the Department of Labour (“L” Division – 8th Floor, Narahenpita).

1. Primary Forms & Letters (Required)

  • Form AMD 1: The formal Amendment Request Form filled explicitly in English Block Letters.

  • Employer Request Letter: An official letter from the employer addressed to the Superintendent of the EPF Department (Central Bank of Sri Lanka), confirming the old/incorrect information alongside the new/correct details.
  • Labour Department Advice: If the previous employer is no longer active or functioning, the member must obtain an official advice/recommendation letter from the relevant District Labour Commissioner or Labour Officer.

2. Mandatory Identity Proofs

  • National Identity Card (NIC): Clear copy certified as a true copy with the official seal and signature of the employer.
  • “B” Card (Form H): Certified copy aligned with NIC details.
  • Birth Certificate: Certified copy required.
